3/5/2023 EnchantaVaughn Approved Behavior: During todayâs visit, client was in a silent and distant mood, as evidenced by her cold demeanor and downward expressions. Once client got the courage to speak, she described that she worries about problems between her family, among other things. QMHP engaged mother in a discussion to determine behavior since the last session. Mother reports as well as client she gets angry when she cannot have her needs met right then and there. Behavior: Client was a little more verbal when talking about things she cared about, like music, games, and social media. Client seemed to switch moods a lot depending on what is being asked. Client continues to display nonchalant behaviors such as shrugging her shoulders throughout the session. Intervention: QMHP monitored client individualized behavior modification through collaboration with mother. QMHP educated and practiced with the client on using breathing techniques and other anger management skills to assist her with maintaining a calm spirit. QMHP provided mom with at home strategies to practice with client in case the behaviors that persists become uncontrollable or overwhelming. QMHP brainstormed with client a list of conflicts that typically occur in her life in efforts to identify possible conditions or triggers that preceded the actual conflict. Intervention: QMHP processed with client to gain a better understanding and identify stimulus that triggered her angry emotions. QMHP reviewed with client conflict resolution skills through instruction, modeling, role-playing, and behavioral rehearsal. QMHP will continue to assist client with managing anger and frustration by reviewing. QMHP demonstrated and provided positive talk therapy as she processed with client about her defiant feelings. QMHP engaged with client on conflict resolution and forgiveness. Response: Client seemed receptive to todayâs individual counseling session in which they verbalized understanding of session. Parent did appreciate client being able to open up about his feelings. She stated she loves all of her children regardless if they donât always treat her the best because as a mother of two itâs not always easy supporting both at the same time. Mom admits she does need to continue improving her parenting skills while trying to balance her personal life and relationship with her kids. Response: Client had positive participation during the role-play activity. Client was able to reevaluate herself and display a more positive attitude toward peers and authority. Client demonstrated expectations and responded well to feedback. Client reported a decreased stress level due to her modulation of social contact, and the success of this was reviewed.
